Apparently, "invitation letters" from everyone who you'll be working with in the U.K. for the maximum 30 day period.  Unknown as to whether that includes the amount to be paid, but I suspect that would be a good idea.

Unless you're able to prove an established relationship with a recognised fashion model agency, engagements by top-tier clients (tear sheets) and an invitation from a recognised UK-based arts/entertainments/education organisation, then I'm afraid you'd be wasting your time applying.
